The questionnaire included a disclaimer to make the participants aware that their answers would be analysed and that their answers were anonymous. It is important to let the participants know what will happen with the answers so that they are confident to answer the questions truthfully.
This is a very good questionnaire Renya, where you've asked a lot of questions to understand what audiences expect to see from a horror; the use of pictures compliment the questions well and make it more appealing as a questionnaire.
